MINOR STUDENT PARTICIPATION AGREEMENT
PARENTAL/GUARDIAN CONSENT, AND RELEASE OF LIABILITY
For students under age 18 only. Under Wisconsin law (Wis. Stat. § 990.01(3)), the age of majority is 18. A minor’s signature alone is not legally binding.
I, the undersigned, hereby affirm and represent that: I am the parent or legal guardian of the above-named minor. I have full legal authority to sign this Agreement on behalf of the minor, and I do so freely and voluntarily. I am not prohibited by any court order or other legal restriction from entering into this Agreement on the minor’s behalf. This Agreement applies to all youth classes, workshops, and activities offered by Tock Custom Sewing & Arts Center, Thiensville, Wisconsin 53092, appropriate for minor participants, including but not limited to: sewing, quilting, embroidery, weaving, fiber arts, felting, knitting, crocheting, painting, drawing, mixed media, collage, and other supervised arts and craft instruction designed for youth participants. Studio equipment used in youth programs and covered by this Agreement may include, without limitation: Domestic sewing machines (age and skill appropriate), Sergers and overlock machines (supervised use only), Hand and machine needles, pins, and related tools, Rotary cutters (supervised use; age-restricted per studio policy), Fabric and craft scissors and shears, Electric irons and steamers (supervised use only), Hot glue guns and adhesive tools (supervised use only), Fabric dyes, paints, inks, and adhesives appropriate for youth classes, Art easels, canvases, brushes, and painting tools, All other youth-appropriate studio equipment as posted on-site
PARENTAL ASSUMPTION OF RISK
ASSUMPTION OF RISK — PLEASE READ CAREFULLY
I, as the parent or legal guardian, acknowledge and agree that participation in sewing, fiber arts, visual arts, and related craft activities at Tock Custom Sewing & Arts Center involves inherent risks of injury or harm, including but not limited to: cuts, lacerations, and puncture wounds from scissors, rotary cutters, needles, pins, and sharp studio tools; burns and scalds from irons, steamers, hot glue guns, and other heated equipment, eye injuries from needle breakage, flying debris, or thread; needle punctures from hand and machine sewing; chemical exposure or allergic reactions from dyes, paints, adhesives, or other studio materials; slip, trip, and fall hazards with the studio equipment; and other risks inherent to creative arts participation by minors. I voluntarily assume all such risks on behalf o my minor child and acknowledge that I am fully aware of these risks in permitting my child to participate.
PARENTAL RELEASE OF LIABILITY
RELEASE OF LIABILITY — PLEASE READ CAREFULLY BEFORE SIGNING
In consideration of Tock Custom Sewing & Arts Center permitting this minor to participate in studio activities, I, as parent or legal guardian, hereby voluntarily and knowingly release, waive, discharge, and covenant not to sue Tock Custom Sewing & Arts Center, its owners, directors, officers, instructors, employees, volunteers, and agents (collectively released parties) from any and all claims, demands, losses, actions, or liability for personal injury, property damage, or death arising from the minor’s participation in studio activities, including claims arising from the released parties own negligence. The word negligence as used in this release is intended in its full legal sense and includes the failure to exercise reasonable care. The release does not apply to, and expressly excludes, any claims arising form the intentional misconduct or reckless conduct of any released party. This release shall not apply to conduct constituting willful, wanton, or reckless disregard for the safety of minor participants. I have carefully read this release, understand its contents, and sign it voluntarily as my own free act and as legally binding agreement.
PARENTAL INDEMNIFICATION
I, as parent or legal guardian, agree to indemnify and hold harmless Tock Custom Sewing & Arts Center and its owners, directors, officers, instructors, employees, volunteers, and agents from and against any and all claims, suits, demands, losses, damages, costs, and attorney’s; fees arising out of or related to: (a) the minor’s participation in studio activities; (b) the minor’s use of studio equipment; (c) any violation of studio rules by the minor or myself; or (d) any claim brought on behalf of the minor related to their conduct at Tock Custom Sewing & Arts Center. This indemnification obligation does not apply to claims arising from Tock Custom Sewing & Arts Center’s own intentional misconduct or reckless conduct.
EMERGENCY MEDICAL AUTHORIZATION
In the event of a medical emergency involving the above-named minor, and if I, as parent or legal guardian, cannot be reached by reasonable efforts, I hereby authorize Tock Custom Sewing & Arts Center staff to: (a) contact emergency medical services (911); (b) seek emergency medical treatment for the minor at the nearest appropriate medical facility; and (c) provide attending medical personnel with the medical information contained in this form. I understand that Tock Custom Sewing & Arts Center is not a medical provider and that this authorization is for emergency situations only. I remain financially responsible for all medical costs incurred.
SECTION 10 — SUPERVISION POLICY ACKNOWLEDGMENT
I acknowledge that I have reviewed and understand Tock Custom Sewing & Arts Center supervision policy for minor students, including: Drop-off policy: Tock Custom Sewing & Arts Center students under the age of 14 must have a parent/guardian on site during classes/lessons. Students age 14-17 can be left unattended with parental consent waiver, Minors must remain within designated studio areas at all times during class. Minors may not operate industrial or commercial-grade equipment without direct instructor supervision. The studio is not responsible for minors outside of scheduled class time. NOTICE: If you do not wish to sign this consent and release on behalf of your child, please speak with the studio director before enrolling. Tock Custom Sewing & Arts Center may, in its sole discretion, discuss alternative arrangements. Your inquiry will be kept confidential.
MEDICAL INFORMATION

If minor student has any known allergies (food, environmental, materials, latex, dyes, or adhesives), it is the sole responsibility of the parent or legal guardian to disclose pertinent allergy information to Tock Custom Sewing & Arts Center.
It is the sole responsibility of the parent or legal guardian to disclose current medications (if needed) and medical conditions relevant to participation to Tock Custom Sewing Arts & Center.
If minor carries an EpiPen, it is the sole responsibility of the parent or legal guardian to notify Tock Custom Sewing & Arts Center.
GRANT OF CONSENT ON BEHALF OF MINOR
I, the undersigned parent or legal guardian of the above-named minor, hereby grant Tock Custom Sewing & Arts Center the irrevocable, royalty-free, worldwide right and license to photograph and/or video record the minor, the minor's voice, and the minor's creative work, and to use, reproduce, publish, distribute, and display such Images for the same purposes described in Part A, Section 2 of this form. I represent that I am the parent or legal guardian of the minor and have full authority to grant this consent. No compensation; right to revoke - I understand the minor will receive no compensation. I may revoke this consent in writing for future uses at any time; however, Tock Custom Sewing & Arts Center cannot retract Images already published prior to receipt of written revocation.
GOVERNING LAW
This Agreement shall be governed by the laws of the State of Wisconsin. Any disputes shall be resolved in the courts of Ozaukee County, Wisconsin. If any provision is unenforceable, the remaining terms shall remain in effect.
